Incident Reports

OEBC is committed to ensuring that a candidate’s performance is accurately recorded for scoring. In exceptional circumstances, if anything has occurred that could reasonably be understood to have detrimentally affected the candidate’s performance or evaluation, the candidate has an opportunity to complete an Incident Report.
 
The purpose of filling out the form is to provide any additional information which will ensure that OEBC has an accurate account of a candidate’s performance.  The candidate is responsible for completing the form individually and may not discuss the exam or the Incident Report with others before completing the form. The completed form is given to the exam staff.

Written Exam:

OEBC administers the Written Exam remotely. If an incident affecting your performance occurs while challenging the exam, please click the ‘Written Incident Report’ button to the left, to download the writable PDF Incident Report and email it to exams@oebc.ca.

  • If remote proctoring – emailing the report to exams@oebc.ca within an hour of signing out
  • in-person – before sign-out

OSCE:

An incident report can be used for different purposes. Mainly, it provides details of the incident to the Chief Examiner in real-time before completing the exam.

If you experience a material event that you believe may have negatively affected your OSCE performance while challenging the exam, you may wish to file an Incident Report.

  • Candidates cannot carry an Incident Report from station to station. However, if a candidate chooses to file one, they will be able to complete it while being sequestered at the end of the exam.
